The irish coast guard (CHC Ireland opf Irish Coast Guard) came by helicopter Sikorsky S-92A, registration EI-ICR, v. no 920051, when to pledge the second helicopter, which picked up seaman requiring urgent medical attention, then flew off to refuel, but crashed from unknown causes approximately 10 miles west from the Black Sod, County Mayo. About six hours after the accident was found one member of the crew, but later in the hospital succumbed to his injuries. Killed all 4 crew members.
