After the Allied landings in Normandy, German ground troops, exposed to devastating attacks by fighter-bombers and fighter aircraft, raised the demand for a more powerful anti-aircraft gun with a calibre larger than 20 mm. The 2-cm-Flak 30 and 2-cm-Flak 38 were no longer suitable for air defence.

Because there was not enough time to develop a new weapon, the German engineers chose to "marry their wits" with the proven 30mm automatic air cannon 3-cm-Maschinenkanone 103 manufactured by Rheinmetall-Borsig on a modified chassis and platform 2-cm-Flak 38. The cannon was nicknamed "Jaboschreck" - i.e. "the terror of fighter aircraft".

The requirement to deliver 3,000 units by March 1945 (Rheinmetall-Borsig 2,000 units and Gustloff Werke 1,000 units) was beyond the capabilities of the German armaments industry. In addition to Rheinmetall, Mauser-Werke (same Rheinmetall model) and Brunserwerke AG (a version with a longer barrel and thus higher muzzle velocity and higher recoil precluded mounting on the Flak 38 platform and the project was cancelled) were also involved in the development and production. Skoda Werke AG (Plzeň-based Skoda) was developing a twin-barrel version, but it did not leave the drawing board.

Data on the actual number of 3-cm-FlaK 103/38s produced cannot be reliably documented, usually about 150 units are quoted. The four-barrelled version - 3-cm-Flakvierling 103/38 - was produced in very limited numbers.

The successful development affected the reliability of the weapon - the vibrations of the 30mm gun were transmitted during firing to the laffer, designed for the 20mm gun, and caused damage to it, besides making it difficult to aim. A redesigned recoil brake to reduce recoil, reinforcement of the stock and its suspension partially alleviated the problem, but the 3-cm-FlaK 103/38 was still prone to failure.
